Sanjeevani Village Health Outreach Programme of Assam

The Sanjeevani Village Health Outreach Programme of Assam is started by Assam Government (Ministry of Health and Family Welfare) to provide health services at the Village level. Sanjeevani is being introduced throughout the state with 80 mobile health units (MHU). Each MHU is being manned by Registration & Measurement Officer, Lab Technician, Pharmacist, ANM and Pilot. Considering early identification, Screening, referral, follow-up and free medicines for effective chronic disease management. Each MHU have a pre-defined calendar and route plan for delivering the services covering 2 villages and a population of 3000 each day on an average and 48 villages in a monthly cycle of 24 days. At present 80 MHU together cover about 4000 Villages every month. With the motive to provide health facilities to all the age group. The aim of this scheme is to provide 24 hours free ambulance facility through 108 and 104 ambulance service and at the same time free medicine to the patient.

  1. Any patient who belongs to Assam state is eligible for getting treatment and free medicine under this scheme

  1. No special documents required for this scheme but for safer candidate can bring following documents with him
  2. Aadhar card
  3. Identity proof e.g. voter ID
  4. Residence proof e.g. certificate from residence authority
  5. BPL ration card
