A Death Certificate is a vital document provided by state government to the nearest relatives of the deceased, stating the date, fact, and cause of death. In urban areas, death certificates are issued by the Municipal Corporation or Municipal Council, whereas in rural areas, the authority is the Tehsildar at the Taluka level or e-Suvidha Centers situated in the Offices of Sub-Divisional Magistrates where the applicant permanently resides in can apply on any working day. The authority at the village level is the Gram Panchayat Office. A Death certificate can be used for Claiming life insurance benefits, Claiming pensions, Settling estates, Cause and circumstances of death, Death and place of interment, Evidence of age, gender, and race, Genealogical information. At last death certificate is a crucial certificate required and every death is to be reported and registered within 21 days at the place of its occurrence in the prescribed reporting forms. Required eligibility and conditions for applying Death Certificate:

  1. Every death is to be reported and registered within 21 days at the place of its occurrence in the prescribed reporting forms. In following cases, persons are required to register
  2. When the Death has taken place in a house then head of the house or nearest relative of the head of the house or oldest person in the family
  3. When the Death had taken place outside the house
  4. In a Hospital/Health Centre, Maternity Home or other like institutions: Medical Officer In charge or any officer authorized by him
  5. In a Jail: Jail In charge
  6. In a Hospital, Dharamshala, Boarding House: Person Incharge
  7. In a Moving Vehicle: Person In charge of the Vehicle
  8. Found deserted in a public place: Headman of the Village/In charge of the local police station

Information required for applying Online Procedure for Application of Death Certificate: 

  1. Name of the deceased
  2. Age of deceased and sex (Male/Female)
  3. Name of the Father/Husband of the deceased
  4. Place of death, Hospital/House (with details)
  5. Date of death
  6. Purpose for which this Certificate is required
  7. Relationship of the applicant with the deceased
  8. Place e.g. City, Taluka, District
  9. Note: No documents required

Application procedure:

  1. The applicant can put up his/her application for registering the death and obtaining Death Certificate at the e-Suvidha Centers situated in the Offices of Sub-Divisional Magistrates where the applicant permanently resides in on any working day. The applicant is given an acknowledgment Receipt for the same that indicates a Delivery Date for his/her Certificate
  2. Alternatively, the applicant may also apply to the Block Development Officer, Executive Officers of Nagar Panchayat, Executive Officer of Agartala Municipality or Medical Superintendent/of State/District/Sub-Divisional Hospitals as applicable, to obtain death registration and certificates
  3. For delayed registration, the issuing authority, at his discretion may initiate Field Enquiry by the relevant Tehsil/Revenue Inspector/Deputy Collector & Magistrate on the merit of the application. After proper verification, the respective authorities would issue the death certificate to the applicant
