The Dindayal Antyodaya Treatment Scheme Card is provided to the people of Madhya Pradesh State. The state of Madhya Pradesh has a large proportion of scheduled castes, scheduled tribe and BPL groups in its population. Lack of treatment facilities especially for hospitalization was a major cause of concern to the families belonging to these groups. The Government of Madhya Pradesh has made a commitment to provide social security to the weaker section of the society. So for that purpose, Madhya Pradesh State has launched Dindayal Antyodaya Treatment Scheme Card on 25th September 2004. Initially, the scheme was for below poverty line families belonging to the scheduled castes and scheduled tribes. The scheme provided Free medical treatment and investigation up to a limit of Rs. 30000/- per family per annum for treatment and investigation in all government health facilities. The benefit is available only to hospitalized patients

Eligibility for Application Dindayal Antyodaya Treatment Scheme Card in Madhya Pradesh State:

  1. Applicant should be Resident of Madhya Pradesh State
  2. All BPL families irrespective of no of members in the family
  3. All members of the primitive tribal groups irrespective of their economic status
  4. Families of holders of “Mukhya Mantri Mazdoor Suraksha Card” & “Nirman Shramik Card
  5. All families which are registered in the BPL list issued by the Panchayat/ urban local bodies are eligible to get the family health card. The names of family members are added on the basis of ration card, certification by competent authority is needed if ration card is not available

Required Documents:

  1. Application on a plain paper
  2. Proof of Birth of the person in respect of whom the certificate is required
  3. Copy of Ration Card
  4. School leaving certificate, if any, showing date of birth
  5. All documents to be attested by a Gazetted Officer

Application Procedure:

  1. For Issuing Dindayal Antyodaya Treatment Scheme Card in Madhya Pradesh Applicant should visit the official website of Madhya Pradesh Government: http://mpedistrict.gov.in/Public/index.aspx
  2. After that applicant will see the following Image
  3. After going on that site Applicant has to click on Dindayal Antyodaya Treatment Scheme Card which is present under the Available Facilities Section
  4. Post applicant will see the following image
  5. Then after Applicant can see Download application form option on the Upper right side of the image where applicant has to click for Dindayal Antyodaya Treatment Scheme Card form
  6. After getting the form, next need to fill it properly with all required and relevant details. Check all details in form after filling application form completely
  7. Now attach required and relevant documents with application form. All required are to be submitted with form
  8. Proceed to concerned authority office and submit your application with all required documents and fee charges to obtain Dindayal Antyodaya Treatment Scheme Card. The concerned officer verifies all details in form and documents. After all verification, he will Dindayal Antyodaya Treatment Scheme Card
  9. The department sends a notification on your registered mobile number or email id. If no notification is received, then visit concerned office after prescribed time limit to collect Dindayal Antyodaya Treatment Scheme Card
