  8. Bride and Bridegrooms 6 joint photographs copies in white or blue background
  9. The groom and bride’s 2 photograph copies in the marriage dress at the time of marriage ceremony celebration was taking place are required to show their involvement in the ceremony
  10. One wedding card copy
  11. Groom and bride’s identity proof (passport or PAN Card or Voter ID card etc.).
  12. Husband’s address proof (Driving license, Ration card, Passport, Telephone bill, House Rent Agreement.)
  13. Groom and Bride’s age proof (mark sheet of 10th standard with the birth date or Passport).
  14. In case the name of the bride has been changed after the marriage then the affidavit is required.
  15. The newspaper in which the information of the name change was published

  18. Name and address of the person who solemnized the marriage (If any)
  19. Place, namely the Village, Taluka and District where the marriage was solemnized, with full address
  20. Particulars of 3 witness

Offline Application procedure:

  2. Fill in the marriage registration form
  3. Submit the filled form to the sub-registrar office of the locality for which you have address proof
  4. The couple has to appear before the Registrar along with their parents or guardians or other witnesses within one month from the date of marriage
  5. All witnesses should sign in front of the registrar
