Online Procedure for Application of Birth Certificate in Odisha

Procedure and documents required for Online Application of Birth Certificate:
Documents  Required:  

  1. Aadhar card/ Driving license/ Voter card (Identity proof)
  2. Residence proof
  3. Application form (Available in Municipal Corporation)
  4. Affidavit in case you have not register after birth occurrence (that affidavit should be about what you are child of your parents)
  5. Marriage Certificate if needed
  6. Proof of birth, letter from hospital where child is born
  7. Parent’s identity proofs for verification
  8. Some processing fee will be there in municipal corporation
  9. A valid Registration number of the child. If he does not have then may contact Health Office for that
  10. Debit/Credit Card to use in Payment Gateway for paying fees of the certificate.
  11. A valid email address and Mobile number, This must be filled while registering with the site as a Citizen User. The email address and Mobile number will be used for sending certificate status

Application procedure:

  1. For online application of birth certificate candidate need to visit: https://www.odishaonline.gov.in
  2. A citizen can apply for the birth certificate by logging in as a Citizen User. For this, one must have a login/User id. If he does not have, then He can sign in for the same and get his login/User id and password. Once he got his login/User id, he is eligible to avail the services in Odisha online portal
  3. For applying Birth Certificate in Odisha online Portal, he must have the following things in hand to proceed
  4. A valid registration number of the child. If he does not have then may contact Health Office for that, the person seeking registration number for the child must be a parent or guardian of the child, Identity proof, Debit/Credit Card to use in Payment Gateway for paying fees of the certificate, a valid email address and Mobile number this must be filled while registering with the site as a Citizen User. The email address and Mobile number will be used for sending certificate status
  5. A citizen who has applied for the certificate can also check his application status in Odisha online Portal. When the application for certificate is approved by appropriate Authority (ULB/Municipality office) User can take the print out of the digitally signed certificate
