Required Documents:

7. Copy of any one of the Ration Card / Duplicate Payment / Telephone Bill / Gas Connection Record / Ownership Deed Rental Notice for certification of the employer/management / partner’s address
8. Shop/installation photograph in which the name of the shop/installation name plaque is displayed

2. Present and permanent address
3. Date of birth, Age
4. Occupation and Designation
5. Old Shop no
6. Address of Shop
7. Copy of any one of the Ration Card / Duplicate Payment / Telephone Bill / Gas Connection Record / Ownership Deed Rental Notice for certification of the employer/management/partner’s address

Application Procedure:

1. For issuing Renewal of Shop Registration Applicant should visit the official website of Labour Department of Madhya Pradesh Government: http://www.labour.mp.gov.in/labourcommissioner/staticpages/registrationdocuments.aspx

7. Then after Applicant has to click on proceed for online registration

10. After that applicant will see multiple options such as New registration, Renewal of old Registration, Renewal of shop Registration
11. So in that option applicant must click on Renewal of shop Registration

14. Now you can see two block 1^st is of old Registration No. and 2^nd is of Password
15. Put that information and go further and follow procedures
