A critical element towards fulfilling the vision of a Cleft Free Assam is the establishment of a comprehensive cleft and child care center which was set up at the Mahendra Mohan Choudhury Hospital, Guwahati for providing surgical care to patients with cleft deformities. These centers are cleft care center matching global standards to provide comprehensive care to patients of Assam having the capability of running 6 operating tables simultaneously

Required eligibility and conditions for applying Operation Smile Free Surgery Scheme for Children Having Cleft Palate and Lip:

1. The candidate child must be a resident of state Assam or his/her parents must be residents of Assam
2. Patient child living in Assam state with untreated cleft lips or cleft palates is eligible or All cleft lips or cleft palates people of Assam state are eligible

Document required for Operation Smile Free Surgery Scheme for Children Having Cleft Palate and Lip:

1. Permanent residence certificate e.g. certificate form residence authority
2. Address proof e.g. Aadhar card
3. All relevant documents related to the illness including expenditure receipts of laboratory tests (viz. x-ray report, doctors advice, pathology test report etc)
4. Birth certificate
5. BPL ration card (if available)
