A Death Certificate is a vital document provided by state government to the nearest relatives of the deceased, stating the date, fact, and cause of death. The certificate can be sued to proof the date of death, individual social, to make free from judicial and governmental obligations, also to settle disputes relating to property inheritance and for the family insurance. At last death certificate is a crucial certificate required and every death is to be reported and registered within 21 days at the place of its occurrence in the prescribed reporting forms. The person who needs this certificate can visit the “e-Nagar Palika website” of Madhya Pradesh.

Required eligibility and conditions for applying Death Certificate:

1. Every death is to be reported and registered within 21 days at the place of its occurrence in the prescribed reporting forms. In following cases, persons are required to register
2. When the Death has taken place in a house then head of the house or nearest relative of the head of the House or oldest person in the family
3. When the Death had taken place outside the house

• In a Hospital/Health Centre, Maternity Home or other like institutions: Medical Officer In charge or any officer authorized by him
• In a Jail: Jail In charge
• In a Hospital, Dharmshala, Boarding House: Person In-charge
• In a Moving Vehicle: Person In charge of the Vehicle
• Found deserted in a public place: Headman of the Village/In-charge of the local police station

Documents required for Death Certificate:

1. Proof of Death of the deceased
2. An affidavit specifying the date and time of death
3. A copy of the ration card for proof of identity
4. In the case of Medico-legal cases.

Application Procedure:

1. For Death Certificate Registration can be done online. For online registration, application form can be submitted through e-Nagar Palika services available at http://www.mpenagarpalika.gov.in/

3. Now click on the death registration link or directly click on the following link for online registration http://crsorgi.gov.in/

5. Now click on General Public Signup and Register yourself
6. Enter the username and password and click on “Logon” Button, if your username and password match with the one already registered.
7. In order to do ‘Death Registration’, click on ‘ Add Death Registration’ button
8. After filling all the necessary details correctly as per the form you have to submit it.

10. You will have to pay the through e-Nagar Palika prescribed fees and upload required documents. After filling that application form you will be given an Application number which can be used later on to check the death certificate status details.
11. Please note that the death certificate will be only issued if the application of death certificate details matches the death records by Hospital submitted to Government.

अब किसी भी आपको मध्य प्रदेश में मृत्यु प्रमाण पत्र के पंजीकरण के बारे में चिंता करने की आवश्यकता नहीं है। मृत्यु प्रमाण पत्र एक महत्वपूर्ण दस्तावेज है जिसे राज्य सरकार द्वारा मृतक के नजदीकी रिश्तेदारों को दिया जाता है, जो कि तिथि, तथ्य और मौत का कारण बताता है। प्रमाण पत्र को मौत की तारीख, व्यक्तिगत सामाजिक सबूत के लिए या न्यायिक और सरकारी दायित्वों से मुक्ति पाने के लिए, संपत्ति उत्तराधिकार से संबंधित विवाद और पारिवारिक बीमा के लिए उपयोग किया जा सकता है। मौत प्रमाण पत्र जो की एक महत्वपूर्ण प्रमाण पत्र आवश्यक है और हर मौत की सूचना और निर्धारित रिपोर्टिंग रूपों में इसकी घटना के बाद 21 दिनों के भीतर पंजीकृत करना जरुरी है। जिस व्यक्ति को इस प्रमाण पत्र की आवश्यकता होती है वह मध्यप्रदेश के “ई-नगर पालिका वेबसाइट” पर जा सकते हैं।

2. प्रत्येक मृत्यु की रिपोर्ट ये निर्धारित रिपोर्टिंग प्रारूप में 21 दिनों के भीतर दर्ज की जाती है। निम्नलिखित मामलों में, व्यक्तियों को पंजीकरण करना आवश्यक है।
3. जब मृत्यु एक घर में हो गई है तो घर के वरिष्ठ या घर के वरिष्ठ के निकटतम रिश्तेदार या परिवार में सबसे बड़ा व्यक्ति पंजीकरण करेगा।
4. जब मृत्यु घर के बाहर हुई हो

• अस्पताल / स्वास्थ्य केंद्र, मातृत्व गृह या अन्य संस्थानों जैसे: चिकित्सा अधिकारी प्रभारी या उनके द्वारा अधिकृत किसी भी अधिकारी
• जेल में: जेल का प्रभारी
• अस्पताल में, धर्मशाला, बोर्डिंग हाउस: व्यक्ति प्रभारी
• चलती वाहन में: वाहन के प्रभारी व्यक्ति
• किसी सार्वजनिक स्थान पर निर्वासित पाया गया: गांव के प्रमुख / स्थानीय पुलिस स्टेशन के प्रभारी

मृत्यु प्रमाण पत्र के लिए आवश्यक दस्तावेज:

1. मृतक की मृत्यु का प्रमाण
2. एक शपथ पत्र जो मृत्यु की तारीख और समय निर्दिष्ट करता है
3. पहचान के सबूत के लिए राशन कार्ड की एक प्रति
4. मेडिको-कानूनी मामलों के मामले में

• प्राथमिकी
• पोस्ट मॉर्टम रिपोर्ट

1. मृत्यु प्रमाणपत्र पंजीकरण के लिए ऑनलाइन किया जा सकता है। ऑनलाइन पंजीकरण के लिए, आवेदन फार्म ई-नगर पालिका सेवाओं के माध्यम से http://www.mpenagarpalika.gov.in/ पर उपलब्ध कराया जा रहा  है।

5. अब जनरल पब्लिक साइनअप पर क्लिक करें और अपना स्वयं का पंजीकरण करें
6. उपयोगकर्ता नाम और पासवर्ड दर्ज करें और लॉगऑन बटन पर क्लिक करें
7. ‘मृत्यु पंजीकरण’ करने के लिए, ‘मृत्यु पंजीकरण’ बटन पर क्लिक करें

9. उपरोक्त सभी आवश्यक विवरणों को फार्म के अनुसार सही ढंग से भरने के बाद आप इसे जमा करे
10. आपको ई-नगर पालिका पे निर्धारित शुल्क का भुगतान करना होगा, उस आवेदन पत्र को भरने के बाद आपको एक आवेदन संख्या दी जाएगी जो बाद में मृत्यु प्रमाणपत्र की स्थिति की जांच करने के लिए इस्तेमाल की जा सकती है।
11. कृपया ध्यान दें कि मृत्यु प्रमाण पत्र केवल तभी जारी किया जाएगा यदि मृत्यु प्रमाण पत्र विवरण का आवेदन सरकार को पेश अस्पताल द्वारा मृत्यु के रिकॉर्ड से मेल खाता है।
