Required Documents:

- In case of being a partner copy of the notarized affidavit of the contract and the status of the farm/company
- Copy of any one of the Ration Card / Duplicate Payment / Telephone Bill / Gas Connection Record / Ownership Deed Rental Notice for certification of the employer/management / partner’s address
- Shop/installation photograph in which the name of the shop/installation name plaque is displayed

- Occupation and Designation
- Old Shop no
- Address of Shop
- Copy of any one of the Ration Card / Duplicate Payment / Telephone Bill / Gas Connection Record / Ownership Deed Rental Notice for certification of the employer/management/partner’s address
Application Procedure:
- For issuing Renewal of Shop Registration Applicant should visit the official website of Labour Department of Madhya Pradesh Government: http://www.labour.mp.gov.in/labourcommissioner/staticpages/registrationdocuments.aspx
- After that applicant will see the following Image1
- After going on that site Applicant has to click on Next Button which is present below on that page
- Post applicant will see the following image2
- Then after Applicant has to click on proceed for online registration
- Then applicant will see the following image3
- After that applicant will see multiple options such as New registration, Renewal of old Registration, Renewal of shop Registration
- So in that option applicant must click on Renewal of shop Registration
- Then applicant will see the following image4
- Now you can see two block 1st is of old Registration No. and 2nd is of Password
- Put that information and go further and follow procedures
Fee for the Renewal of Shop Registration: Rs.100 to Rs.500
