The Death certificates are provided to the people of Delhi. Death Certificate is important certificate because it shows the date and fact of one’s Death. Death Certificate is a vital document provided by the government to the nearest relatives of the deceased, stating the date, fact, and cause of death. The certificate can be documented to proof the date of death, individual social, to make free from judicial and governmental obligations. At last death certificate is a crucial certificate registered within 21 days.
Eligibility for Application to Issue Death Certificate in Delhi:
- Every death is to be reported and registered within 21 days at the place of its occurrence in the prescribed reporting forms. In following cases, persons are required to register
- When the Death has taken place in a house then head of the house or nearest relative of the head of the House or oldest person in the family
- When the Death had taken place outside the house
- In a Hospital/Health Centre, Maternity Home or other like institutions: Medical Officer In charge or any officer authorized by him
- In a Jail: Jail In charge
- In a Hospital, Dharamshala, Boarding House: Person In-charge
- In a Moving Vehicle: Person In charge of the Vehicle
- Found deserted in a public place: Headman of the Village/In-charge of the local police station
Required Documents:
- Proof of Death of the deceased
- An affidavit specifying the date and time of death
- A copy of the ration card for proof of identity
- In the case of Medico-legal cases.
- FIR
- Post Mortem Report
- Form-2 given By Concerned Police Authorities

- After clicking on above link applicant will see the following image
- After that applicant should have to click on any of the Municipal Corporation which he belongs to
- After clicking on that candidate will see following image
- Then applicant should click on online services which mention below right-hand side
- Candidate will send to the following image
- Now to get the Death certificate applicant should register online himself and follow further procedures

- The applicant can visit the office of Sub-Divisional Magistrate in whose jurisdiction birth took place, during 9.30 a.m. to 6.00 p.m.
