- Applicant must be the resident of Haryana State
Required Documents:
- Filled Application form
- Proof of Birth of the person in respect of whom the certificate is required
- Affidavit specifying place, date and time of birth/death of the person
- Copy of Ration Card
- School leaving certificate, if any, showing date of birth
- All documents to be attested by a Gazetted Officer

- After reaching to the site candidates should click on Birth certificate which is in services section which is scrolling upward
- After that applicant will switch to next page
- On that page, applicant can see Download forms and Instruction
- Applicant has to click on that
- Here on this page several forms are placed candidate has to click on Birth certificate and he will get form
- At last, applicant should download form and filled it which will be sent to The District Registrar, Births and Death, Haryana
